Kotara Family Practice is located in WESTFIELD Shopping Centre, Kotara.

You can find us on the Ground Floor at the David Jones end of the shops, opposite the new Scott + Dibben Pharmacy. 

There is plenty of parking directly outside, including an abundance of disabled parking spaces, and parking on the upper levels offers direct lift access to the front of the practice.

What to tell us when you make an appointment

In order for your reception team to assist you in the most appropriate way, they will ask you to tell them a little bit about your concerns or health need. This is so they can determine how urgent your need is and how much time you may need with your GP or nurse.
When making an appointment, please provide reception with as much information as possible so they fully understand your needs. If your concern is private, we understand and respect your right to privacy. In this case, please let reception know if your need is urgent and if you feel you will need a longer appointment with your clinician.

Please be aware that online forms are sent via email and may not be secure - If your enquiry includes any specific personal health information please call the practice instead. The practice does not store your personal information - once your enquiry is received and has been acted on your email is deleted from the system. However, if your enquiry relates to a specific health condition your email may be copied into your personal health record.

Parking

There is plenty of parking within Westfield directly outside the practice and in the upper levels of parking at that end of the centre. Each of the 3 upper levels offer lift access to the ground floor, opening directly in front of the practice entrance. There are many disabled spaces available on the ground level as well as in upper levels of parking. 

Parking at Westfield is FREE for the first 3 hours. If you hold a disability parking permit you can have your parking extended by taking your parking ticket and disability parking permit to the concierge desk on level 2 (outside JB HiFi and Toys R Us).
